1. for grades: proceed to PUP what you need
a. Notice of Admission (NOA) and/or
b. Valid ID
2. for board rating (Php75) and board passing (Php75) what you need:
a. yellow paper/verification slip and
b. metered stamp (Php42)
3. for oath form: what you need:
a. oath form, notary (second floor, Php40),
b. documentary stamp (Php21),
c. new cedula (original don’t buy outside PRC it’s fake),
d. picture colored passport size plain white background and complete name tag, paste
4. for registration and board certificate (yung parang diploma makukuha after 6 months): what you need:
a. oath form,
b. cedula,
c. two (2) passport size,
d. 1 pc. 1” x 1” picture colored with white background and complete name tag,
e. 2 sets of metered documentary stamps,
f. one short brown envelope with name and profession, paste
(initial registration fee - Php600, annual registration fee for 2008-2011 – Php450)
REGISTRATION WILL START AFTER THE OATH TAKING. Alphabetical schedules FOR REGISTRATION are now available at PRC and at PUP.

can you clarify what do you meant if you say documentary stamp, metered stamp, and metered documentary stamp...im confused..

*for the passport size pic, does it need to have a white background and a name tag?
*P600 initial then P450 (2008-2011), so I need to prepare P1050?
