1. Duly accomplished Oath form or Panunumpa ng Propesyunal
2. Current Cedula
3. 2 pcs. Passport size pic (colored, w/ white BG & w/ complete nametag)
4. 1 pc. 1x1 pic (colored, w/ white BG, w/ complete nametag)
5. 2 sets of metered documentary stamps
6. 1 short envelope w/ name and profession
7. 600php initial registration fee
8. 450php annual registration fee for 2008 - 2011
* successful examinees should personally register and sign in the roster of registered professionals.
