TO: ALL November 2008 NLE Passers
SUBJECT: REGISTRATION FOR THE ISSUANCE OF LICENSE
1. Mass Registration starts on Monday, March 2, at the Regional Office from Monday to Friday, 8:00 a.m. to 5:00 p.m.
Only Baguio examinees will be accepted for registration. “Register Where You Took the Exam” policy is applied. (Office Order No. 2009-90 dated February 19, 2009)
2. REGISTRATION REQUIREMENTS
REQUIREMENTS:
Duly accomplished Oath form or Panunumpa ng Propesyonal
Registration fees- P 1050.00 for baccalaureate, P870 for non baccalaureate
Current Community Tax Certificate (cedula)
2 copies of recent passport size pictures with complete name tag, colored white background
1 copy 1x1 colored picture, white background with complete name tag
One Brown Envelope WITH Php 50.00 worth of mailing stamps, with exact amount of P21.00 for the Documentary Stamp if you wish to receive your certificate (lupon) through mail (OPTIONAL)
IF REGISTRANT IS ABROAD AND WISHES TO REGISTER THROUGH A REPRESENTATIVE, SHE/HE MUST SUBMIT THE FOLLOWING ADDITIONAL REQUIREMENTS:
Special Power of Attorney from the Consul/Notary Public abroad
Oath form duly filled up by applicant and notarized by the Consul/Notary Public abroad.
Acknowledgment from the Consul
Photocopy of the passport indicating the port of entry
STEPS AND PROCEDURES
Present duly accomplished Oath form to the Records Officer (Window 14) for record verification of Name, Birth Date, Date and Place of Exam and ratings
Pay the prescribed fees at the cashier then proceed to Customer Service Center for documentary stamps
Submit properly accomplished Oath Form together with the above mentioned requirements including Official Receipt (OR) at Window 12
Fill-out and sign in the registry leaf and wait for your claim slip/stub.
IMPORTANT NOTICE:
TO ALL REGISTRANTS: You are required to take your oath of profession before you register for your license. If you failed to attend the mass oath taking ceremony there is a special oath taking for FREE scheduled at every Thursday 11:00 am, at the Office of the Regional Director. Please register at window 11 for your attendance.

sir/madam: good morning!!! i am from ilocos norte.. a board passer of NLE 2008. i just want to inquire regarding the schedules of the regional oath taking at laoag. ive read some info that we can now register for our license starting on march 2, 2009 and one of the announced requirement is the accomplished oath form.. it also says there that we cannot register unless we have participated the oath taking ceremony. how can we register on march 2, 2009 if the schedule of our oath taking here in ilocos norte will be on april 11, 2009 @ fort ilocandia? quite confusing sir/madam. please provide us clearer info regarding this because many passers here in ilocos are asking where can we find for our schedule in registering at baguio beacuse only manila area has the schedule yet. do we have schedules in registering by surname? where can we get the oath form? there are rumors also that we are going to have our reservation at MARIANO MARCOS MEMORIAL HOSPITAL & MEDICAL CENTER for our oath taking. who are the officers to be approached? what are the requirements we are going to provide to them? looking forward to your reply... Good day!
I passed my NLE 2008 this November, but my problem is Im here in Italy,my registration date is on March 9,2008 but I can't be there on that day, so my question is when is the last day of registration and the last passing day of oath taking form.pls i need the reply.thank you!

Hi!!!!
I'm From Bacolod and I took my Board exams in Cebu last Nov 2008.....and I passed ( Thank GOd!) but we now have an issue regarding our Oath Taking Ceremony because we, the Cebu NLE Passers are not allowed to have our Oath in Bacolod.....but instead, we are advised to go to Cebu. What we are worried about is that we will spend additional amount of money to file for our registration for PNA and the Oath...then back to bacolod, then on d 15th of April, we will travel back to Cebu for the Oath...Many are worried because not all can afford the back and forth travel to cebu....and another issue is our families cannot witness the event...as we all know..Oath taking is the much awaited event anyone is waiting for....so the families are eager to attend the event...however if our oath would take place in Cebu, the family members couldn't attend because of the additional expenses..can you help us with the issue??? atleast try to help us convince or ask the BON to send a letter to Bacolod to allow the Cebu takers To have their oath in Bacolod....thanx...please!!!!
